Medicare Beneficiary & PPO/Insurance
If you are a Medicare Beneficiary with a physician’s referral for medically necessary PT treatment, we are unable to provide care. We are no longer enrolled as a Medicare provider, and our billing system is not set up to submit claims to Medicare. While we cannot accept payment (including self-pay) for medically necessary PT services, we may still be able to see you for general fitness, maintenance or wellness services. These services are not considered Medicare-covered interventions and may be available, even if you have secondary insurance.
We also are an out-of-network provider but many PPO plans reimburse a portion of PT with a physician’s referral. If you’d like to submit for reimbursement, we’re happy to provide a super bill that includes all the codes your insurance needs. Coverage may vary by plan, so please check with our insurance plan‘s coverage.

Philip earned his Bachelor’s Degree in Physical Therapy from the University of Alberta, Canada, in 1994, with specialized studies in Sports Medicine and Orthopedics. He later obtained certification in Manual Therapy and earned his Doctor of Physical Therapy degree from the University of St. Augustine for Health Sciences. With over 30

With over 30 years experience, Philip continues to be committed to professional growth and excellence in patient care. He has built a diverse and extensive clinical background working in acute care, outpatient rehabilitation, and home health care settings. Outside of his professional career, Philip enjoys spending time with his family and competing in USTA league tennis.

Jeffrey earned his Master’s Degree in Physical Therapy from Quinnipiac University in 2001. After completing his degree, he relocated to Naples to be closer to family.
Since becoming a licensed Physical Therapist, Jeffrey has developed extensive experience in outpatient rehabilitation, treating a wide range of orthopedic and neurological conditions. His clinical expertise includes the treatment of neck and back pain, upper and lower extremity injuries and joint disorders, balance and gait dysfunction, neurological impairments, post-operative rehabilitation including joint replacements, and various musculoskeletal conditions. In addition, he brings more than 20 years of experience in home health care and healthcare management.
Outside of his professional practice, Jeffrey enjoys coaching his children’s baseball and soccer teams.

Michelle graduated as a Physical Therapist Assistant in 1998 and later earned her Bachelor’s Degree in Health Care Management. With more than 28 years of experience in the physical therapy field, she has provided care across a variety of settings including outpatient rehabilitation clinics, assisted living facilities, independent living communities, and home health care.
Michelle currently specializes in home health services and also provides individualized aquatic therapy programs conducted in patients’ home pools, allowing treatment plans to be tailored to each patient’s unique goals and functional needs.
She is passionate about helping her patients achieve lasting success through improved health, wellness, and quality of life.
